It should be easy to verify if this is a real immigration form for the
purpose it states here. Just call the Federal Information center, and this
number is listed in the blue pages of every  local yellow pages throughout
the country. It is as follows:

1 800 688 9889.

You can find out almost anything you want to know that has to do with
services and goods offered by the Federal government, from tax questions, to
goods auctioned by the Federal government, and where those auctions are held
in your local area.

 I called and was given another toll free number where you can order any INS
form. I called this number, and sure enough, my order for this form was taken
over the phone, and it is free of charge. The automated ordering system told
me that i will receive this form in 7 to 10 days. I will let you guys know if
it is the form in question, and what it is for.
BTW, this number for ordering INS forms is as follows:

They also gave me the address of the INS website where you can look up any
form and other information, and it  is as follows:

It said the site was timed out when I tried to access it, but you guys can
try it.
