The following message and questionnaire is only for those of you who receive
Urgent Actions for an Amnesty chapter or student group. Please do not

you are an individual UA activist not affiliated with a group.

We need your help! If you are member of an Amnesty chapter or student group,
please ask your group coordinator if they have filled out "Vital Signs:
1999-2000 Group Report" (the group evaluation form). If not, please work
with
your group to fill this important form out. We have extended the deadline to
May
17, so you still have time to fill it out! Please do so soon.

The following outlines four different ways that you can fill the form out
electronically.
1.   The easiest way is to fill it out in the Members section of the website
(www.amnestyusa.org/members). If you do not have a login name, use "amnesty"
and
the password "viaduct".

If you would like to save a copy for yourself, please choose one of the
following three electronic options:
2.   Copy this e-mail into a new e-mail, answer the questions and send it to
[log in to unmask] Answer each question by putting an ?x? next to the
appropriate
answer(s) and by typing in your comments.
3.   Download the form from this e-mail (the form is in MS-Word for
Windows),
answer the questions, save it and send it to lsock@aiusa as an attachment.
4.   Print this e-mail out, fill it out and send it by MAY 17, 2000 to:
