Evian, I think ALL of MRC in Gambia should be Gambians. And they should come under the Department of health. What baffles me is why from the inception of MRC in Gambia up to now, the entire staff is not Gambian? From the security guard to the Director. When I become President of Gambia, I will transform MRC into a college of tropical medicine.
It is unconscionable for another nation or government to conduct research in Gambia and on citizens of Gambia. Don't worry all Gambians will not die when there is no longer an MRC. Haruna.

Suntou, I also used to ask the same question and luckily for me I was reassured by a personal friend working for MRC Gambia that although the MRC is largely a Research facility, they are bound by the same codes of conduct and ethics generally applicable to members and institutions of the medical profession. Moreover most of the employees of the MRC in the Gambia are Gambians who I believe would not be a party to any unethical medical or research practice, of which there is yet to be any such evidence. Base on close experience of MRC patient care in the Gambia, I think they are a good partner to Gambians in the field of both medical care and reseach. MRC is a research facility; with emphasis on tropical infectious diseases in the case of the Gambia.
I don't think people die at MRC clinics in the Gambia because of "untrained" doctors ( whatever that means) as you put it, but due to the advance stage of their ailments before they seek health care.
Is it reasonable to assume that MRC employ residents fresh out of Med school or even med students to work in their clinics in the Gambia. You betcha. That happens in every country. I work in the health care field in addition to my IT career. I have worked with residents and med students in hospitals in the united states. Granted they are supervised and I believe that MRC has the same procedure in place in the Gambia. Anything short of that will be malpractice.
I know they use to conduct blood test for malaria patient and some other sicknesses, what need knowing now is for them to clarify whether they also double as a hospital. Many seriously ill people according to reports prefar the MRC to RVH, may be this could be due to cost or somethimng i don't know.
Research hospitals or clinics like the MRC do treat patients. Usually they are at the cutting edge of technology in health care. Therefore,MRC treating patients is just an extension of their research into tropical and other infectious diseases.
