Has Ryan been tested for allergies?    He could be allergic to a food or even the material that his clothing is made from.  My daughter was on a totally restricted diet after the elimination of wheat, gluten, milk and related products.   She could only tolerate rice cereal, apple products and lamb.  We slowly added one food at a time until we came up with a workable list of things she was able to eat without a problem.  Two months of intensive testing in the hospital and several years of working with a pediatric gastrointestinal expert led to this result.   We also had to eliminate all talc, latex and some other materials from her environment.   Difficult to eliminate the talc in school once she started attending preschool, but we managed by sending in a list of the things she could tolerated eating or being in contact with during the day.   Most teachers were cooperative over the years and now that she is an adult she is able to avoid on her own things that cause a problem for her.
