Virtual Private Networks

One of the most currently popular areas for securing and transmitting
healthcare information is the use of virtual private networks (VPNs) to
transmit data via the Internet. Utilizing “private” networks of the
Internet through a wide area network (WAN) backbone, long-distance dial up and
leased line charges can be eliminated (Guy, 1999; Hurwicz, 1997). Connections
to the Internet-based VPN are local. In fact, VPNs have been reported to lower
networking costs from 30 to 80% (V-One, 1999).

In addition to lowering network costs, Internet-based VPNs have the
ability to extend worldwide access with local access capabilities provided
by the Internet. This accessibility can improve productivity easily and
affordably while delivering high reliability, redundancy and resiliency of the
Internet (Bay Networks, 1999). VPNs can also increase flexibility and simplify
healthcare operations with a single per-site connection to an intranet,
extranet, and the Internet (Guy, 1999, ).

One likely VPN application for health care is the extranet. An extranet
is a network among multiple, independent parties. For example, a
practitioner can use an extranet to access patient records and Medicare
claims processing.

Other organizations may benefit from intranets within their own staff and
offices. For instance, hospital staff can use the intranet to securely access
patient records from their homes or offices or satellite clinics can use
the intranet to communicate with the central hospital. VPNs allow
practitioners to be able to securely access:

        Master Person Index: A mechanism for identifying patients across
        disparate systems
        Clinical data repositories: A complete clinical patient history
        Global registration: Patient registration at any point in the
        health care history
        Referrals, claims, lab orders and results, and benefits administration"
        (Guy, 1999)

VPNs also utilize the latest developments in communications security.
Specifically, VPNs use encryption technology and so-called tunneling
techniques to ensure that communication that travels through intranets,
extranets and the Internet is protected (Guy, 1999; Hurwicz, 1997). VPN security is
provided by identification of the user to prevent infiltration from outside
the network; maintenance of the privacy of the data as it passes through the
network;and selective access to different network assets for different users.

Satelife & Netaid have Joined Forces

The United Nations Development Programme (UNDP) and Cisco Systems have
joined forces to create NetAid, the beginning of a new, long-term effort to
utilize the unique networking capabilities of the Internet to promote development and 
alleviate extreme poverty across the world. It is the largest public/private 
partnership ever to take place to address this serious issue.

The NetAid Foundation is a global exchange point to link people to suc-
cessful agents and agencies of change in order to help the millions of
men, women, and children who don't live on the cutting edge, but who
instead live on the edge of survival.

Where does SATELLIFE fit in? SATELLIFE was highlighted in the UNDP 1999
Human Development Report for its track record in using technology to
improve health in poverty-stricken areas of the world. NetAid is now
featuring SATELLIFE as one of the select organizations around the world
that successfully harnesses the power of the Internet to help solve the
global problem of poverty. SATELLIFE is excited about this initiative
and that people everywhere are recognizing that the Internet can be
used on a grand scale to alleviate human suffering.

Experimental Electronic Research Archive 

The Lancet has launched an experimental electronic research archive (ERA)
in international health.  This self-archive will be owned by authors and
administered by The Lancet.  Access will be unrestricted through the ERA
website, http://www.thelancet.com/newlancet/eprint/ Their objective is to
create a searchable electronic public library of research in international
health. They are seeking content that covers all issues relevant to
medicine in the developing world.  Please support this excellent effort to
make knowledge accessible to all members of the international health
community.

A  Telehealth request for assistance from the South Asia Listserv
5 projects have been identified at Dhaka. Of these, a funding proposal
prepared by Imran Rasheed, Irfan Khan, Samudra Haque and myself is already
under examination. This proposal incorporates some elements of the Internet
radio and satellite-based content distribution, and telecenter proposals. We
are looking for sponsors for the optic fiber backbone seminar, and I am
preparing a funding proposal for an e-commerce study that will look for
ways to ensure that the poor are not excluded. That leaves the telemedicine
project.

Is anybody willing to prepare a project proposal including budget, for a
study or actual implementation, and willing to spend the time and effort to
see it through? If yes, please let us know quickly what you have in mind,
and a timeframe in which we can expect the proposal.

A Request for Assistance

I am trying to identify telemedicine projects relevant to pediatrics. I have
heard presentations from some university-based projects, but I'd like a more
comprehensive list. If you know of any other peds-related telemedicine
projects (real or proposed) please let me know. I am compiling a list of
projects as part of an AAP project on telemedicine needs of children.

Rural Healthcare in Vietnam

Health Information Waystations

With input from WHO, World Bank, and leading health NGOs, development 
has begun on a 5 year project to upgrade 1,000 existing health information 
resource centers throughout the developing world into digitally-enabled 
Information Waystations (IWs).

Project would supply computer, CD-ROM drive, databases, modem, global
satellite telephone & Internet link, and includes training, maintenance, 
technical support and development of individual sustainabilility
