Telemedicine Reimbursement Takes a Significant Stride Forward

Legislative language increasing Medicare reimbursement for telemedicine has passed both the United States Senate and House of Representatives. It now awaits signature by President Clinton, who has indicated that he will sign the measure. 
Passage of the language, originally opposed by the Health Care Financing Administration (HCFA), is a clear sign that Congress has embraced the continued expansion of telemedicine. The original bill is the result of efforts by Senator James Jeffords (R-VT), and the support and work of Rep. John Thune (R-SD). 

The legislation substantially improves the existing Medicare program providing coverage of telemedicine and serves as a major step forward in expanding such services throughout the U.S. Among the most significant improvements, the language: 
·       expands the eligible service area from rural health shortage communities to all non-metropolitan counties and existing urban    Medicare demonstration sites 
·       expands the eligible medical services that are included in the program 
·       eliminates the current fee split requirements between presenting health professionals at the patient and referring sites 
·       includes a new $20 facility fee 
·       introduces eligible store-forward services for Alaska and Hawaii

The measure also clarifies that telehomecare is an eligible service under the new Prospective Payment System that governs use of Medicare dollars for homecare. 

The bill is slated to go into effect October 1, 2001. HCFA will most likely issue rules governing changes in the program.

Medicaid Funding Now Available for Telemedicine in 17 States 

According to a June report by the HCFA, 17 states are now using Medicaid funds to pay for telemedicine, indicating increased funding available for telemedicine services in the U.S.  Medicaid reimbursement for services furnished through telemedicine applications is available, at the State's option, as a cost-effective alternative to the more traditional ways of providing medical care (e.g., face-to-face consultations or examinations).
