History (how the patient has been doing) is 85% of any diagnosis, the exam 10%, and the labs are worth (at most) 5%. There is a very wide range of factors that can make weight loss difficult or impossible. The most likely, it seems, include: the wrong dose of thyroid, the wrong kind of thyroid, problems with your adrenals or ovaries, insulin resistance, and nutritional problems to name a few.
